It Asks People to Log In
The most frequent issue: the hosted PDF isn't publicly shared. Scanners hit an access-request or login wall. Fix it by setting the file's sharing so anyone with the link can view, then test with an account that isn't yours.
The Link Leads Nowhere
If the PDF was moved, renamed, or deleted, a static code now points to a dead link. For static codes, the file must stay put. If you need to move it, that's a strong reason to use a dynamic code you can repoint.
The Code Won't Scan at All
